The specific-H&P is the ScyMed's new concept. A specific History & Physical Exam is the H&P for a particular disease or problem. It is a clinically coherent unit that incorporates the specific signs & symptoms for the particular disease or problem, as well as the pertinent positive and negative data.

The organization of the eH&P™ is a very innovative one. We have defined more than 1500 eH&P™ Elements (or Data elements). An eH&P™ element can be any H&P section, anatomical area, symptom, sign, clinical maneuver, technique, clinical finding, etc. that may be part of the History & Physical examination. eH&P™ elements can be 'generic' or 'specific'.

'Generic' elements are always in 'black color'.

They represent H&P entries that are always included in the H&Ps (like heart sound 2).

'Specific' elements appear in 'red color'

They represent H&P entries that are included in the H&Ps of certain diseases, not all. Like the paradoxic splitting of the heart sound 2 or Hepatomegaly.

The Specific-H&Ps may be created in different formats (Print and Read).
The Print format is a concise, small-font, highly organized, one-page format. The read format is more expanded and has bigger fonts. The Print format may be color-coded or black.

The Color-coded format is a unique and innovative feature that displays the disease-specific signs & symptoms in red color (they also print in red). For example, polydipsia, polyphagia, polyuria appear in red in the Diabetes-Mellitus-Specific H&P, and so the rest of the signs, symptoms and findings that are specific to diabetes. This innovative, copyrighted and unique color-coded system is great for students and young professionals, it allows for a quick and reliable identification of the data (entries) that are 'common to all H&Ps' (generic) and the data that are 'specific' for a particular H&P.

eH&P™ offers a unique medically-oriented Spanish translation. The Clinical Methods sections as well as the Specific-H&Ps can be translated to spanish.
This translation is color-coded and in-line (in the same text line). English data of the specific H&P appear in black color and the spanish translation follows immediately in blue color (and in parentheses), e.g: Murmurs (soplos).

eH&P™ is incredibly useful for healthcare delivery, teaching, learning and research. Its applications are multiple.
Example 1. A Patient with the diagnosis of Myocardial Infarction arrives to the unit. The student is expected to perform the H&P. With this program the student can quickly create a complete Myocardial Infarction-specific H&P (with his/her name) and Print it before the encounter. This assures a custom, thorough, systematic evaluation. It helps the Student to integrate all the clinical signs & symptoms and to bridge smoothly from books to actual patients. If the Patient is hispanic, the MI-specific H&P can also be spanish-translated to help with the clinical interview.

Example 2. Up until today, in spite of very precise, widely accepted standards of care, the follow/up of Patients is very inconsistent. The out-patient follow/up of a Diabetic Patient (glucose, Hemoglobin a1c monitoring, microalbuminuria, ophthalmology consult, etc.) varies tremendously from Physician to Physician, Hospital to Hospital, City to City, State to State, etc. The follow-up is inconsistently done by different healthcare professionals and institutions. Charting as well as information capturing is also inconsistent and anarchic. As a consequence, morbility and mortality rates are statistically higher than they should. eH&P™ offers a unique solution.
A standard, customizable, structured-capture, System to generate Disease-specific and Patient-specific Follow/Up charts (that can be printed and up-dated from any computer). The system is browser-based (& paper-based -for now-) making it very easy to deploy and avoiding confidentiality and privacy issues.
